The Outcomes Capability Planning and Assessment tool supports providers to develop capability in capturing and using information, and improvement in providing evidence of:

• client outcomes that are realised through their services
• the effectiveness of their services.

The tool aims to build providers' capability in meaningfully reporting on effective results and developing good outcomes data.

This requires:

• identification and measurement of outcomes (what)
• clarity of purpose (why)
• a system to capture the data (how)
• people to operate the system (who).
